CLIENT FORMS

Each client will need a DEMOGRAPHICS page (you only need the REFLEXOLOGY DEMOGRAPHICS if you want reflexology) and an INTAKE PACKAGE. For legal purposes, a minor is anyone under the age of 18 and will need a parent or legal guardian to sign, unless proof is submitted of emancipation or other exceptions. 

*Please note that Barefoot Therapy does not offer prenatal or infant services at this time.
